The Affidavit for Temporary Custodian’s Healthcare Authorization is intended to provide parents/guardians with a way to name a temporary custodian who can serve as a healthcare decision maker for their minor children, on a short term basis if the parent/guardian cannot be reached.  Such authority is limited to what is specified in 13 Del. C. § 707(b)(5).  This Affidavit may assist parents if, for example they are unavailable due to their own emergency, such as COVID-19 illness, and want to ensure another trusted person can serve in this role.
